Outreach Services

Outreach Services deliver support to our clients where they need it, in their homes and in the street.  CDP Outreach Workers travel to meet those in need wherever they are in our area.

The CDP develops services in consultation with service users in a needs lead approach.  This has successfully helped us to develop a range of different outreach support and aftercare services.

These services provide interventions for people prior to, during and after treatment.  They particularly target hard to reach and stigmatised groups of clients.

These services also require significant liaison and joint working arrangements across the independent and statutory sectors.  We wish to offer our thanks and appreciation to the numerous hosting agencies in the East Riding who have enabled the CDP to provide these services.

From April 2004 to March 2005, we saw: 264 East Yorkshire residents
